Certified Nurse Aid - CNA

POSITION SUMMARY

Provides personal hygiene care to hospice patients following the patient's Plan of Care.      

ESSENTIAL FUNCTIONS

• Provides personal hygiene care to patients according to the individual Plan of Care, such as bathing and shampooing.     
• Deliver routine medical supplies as needed to patients.     
• Observe and report any changes in the patient's condition to the Nurse/Care Manager.    
• Demonstrate respect and understanding of the philosophy, mission, and service standards of CHOT and for providing excellent customer service.     
• Successfully complete 12 hours of in-service training annually.    
• May assist patient with transfers as needed.    
• Provide compassionate companionship to patients while delivering services.    
• Depending on the census, assignments may include patients in the field or inpatient units.         

POSITION QUALIFICATIONS  

• HS Degree or GED    
• Proof of State Certification - Certified Nurse Aide    
• 1 year of direct patient care experience required   
• Demonstrate skills and written competency prior to working independently with the patient    
• Able to operate the cellular device
